Famiglia translates to ‘family’ which marks the first chapter of Mafia: The Old Country. This segment is straight-forward and undemanding though Enzo will be tantalized by a possible love interest, the Don’s daughter. The following guide walks through the most primary mechanics as well as collectibles, Charms, miss-able dialogue options, and other points of interest.

After Enzo disfigures II Merlo’s face, he makes a break for it. Enzo attempts to find refuge in a random farmhouse only to be disturbed by the sounds of vehicles pulling up. Apparently, II Merlo was able to track him down by spotting Enzo’s getaway horse outside. Here, the player is tasked with sneaking back to his horse to hopefully escape once again.
Sneak Back to Your Horse
On console, “Instinct” is activated by holding UP (⇑) on the directional pad. As seen in previous installments of Mafia, the player can use stealth and tactical combat to gain an advantage over hostiles. From Enzo’s bed, the player must make their way back to the getaway horse without being detected. Detection will prompt mission failure, take down guards to avoid.

Below, there will be a single guard to practice a stealth takedown with. The guard will generally stay facing the door with no points of interest in the area. Generally, as the player approaches a prompt may appear on the screen with an action button SQUARE (PS5) or ‘X‘ (XBox), to activate a takedown.

Remember, a sharpened knife is lockpick ready so be sure to sharpen them often with SQUARE (PS5) or X (Xbox). It’ll cost 1 whetstone to file a knife so looting bodies after takedown is crucial.
After unlocking the door (with a sharpened knife), vault over the wall and take a backroad towards the getaway horse. Follow the path, go through the partial structure with an opening until the path bends. After the turn there’ll be another one of II Merlo’s thugs facing away. Take him down and loot his body.
Obviously, there will be no more points of interest as the path leads to an arched partial structure and another thug that’ll need to be taken down.
From here, two more thugs will briefly converse over Enzo’s whereabouts with one leaving shortly after. Take these two down either passively or violently. From there, head towards the large double doors and open which will prompt a formidable cutscene.
Ride With Luca
Next, after Enzo is saved by the stately Don Torrisi, he is offered a job in exchange for trespassing/saving his life. The ride with Luca is unambiguous with no points of interest outside of the dialogue.
Main Conversation Points:
- Enzo accidently trespassed onto Torrisi territory
- Torrisi spared Enzo due to his hatred for II Merlo (Damiano Bastoni)
- Torrisi “offers” Enzo a “job”
- The Torrisi Vineyard is described as “more or less” the family business
- Enzo is granted an old, unused pigsty as a place to stay
The scenery is beautiful and leads to Don Torrisi’s villa where the player meets several prominent characters.
Another cutscene cues in as Enzo get’s more acquainted with Don’s Torrisi’s nephew Cesare.
Find Cesare
Briefly, a cutscene plays of a sleeping Enzo only to be triggered awake by Cesare. Cesare orders Enzo to change clothes then gives him several rundowns of the ordinances of Torrisi’s estate thereafter. There will be a healing bandage available to add to the player’s inventory that can be prompted by the Down (⇓) button. A collectible note is located across from the wardrobe that contains detailed grape harvesting instructions.

Help Cesare and Valentina
Meet Cesare outside as he expresses the importance of that night’s dinner. The next point of interest will be a note alongside the crates that entails of a wine delivery schedule, these names may come up later in the story.

A total of three crates need to be lugged to the cart with no points of interest. When finished, a dialogue bubble will appear at Cesare, who tasks the player to help a lady named Valentina. A dialogue icon will appear on her back that can be activated from behind her. She welcomes Enzo but warns him of Torrisi’s temper before delegating her tasks.
Next, the player is tasked with following Valentina briefly into a food cellar where a pig carcass is on the table. Another note from across will prompt a Journal Entry.
The note is a thank you letter from a gentleman named Signor Gulotta. The letter congratulates Don Torrisi on the celebration of his daughter’s name day. Culturally, a name day is a celebration associated with the feast day of the saint after whom a person is named. The letter mentions a gift of a Nero dei Nebrodi, a Sicilian breed of pig, which explains the carcass.
Valentina also explains the importance of never stepping foot inside the villa unless invited and as long as he works, he eats. From there will be two points of interest in the kitchen that will prompt Journal Entries.

Clean the Stables
Valentina thanks Enzo before dismissing him back to Cesare. He expresses suspicions about Enzo as well as disdain for Damiano Bastoni (II Merlo). Cesare leads the player to the stables and tasks Enzo with horse manure clean-up duty. A note will be on the right-hand side of the entrance which details care instructions for Cosimo. There is also a mention of a “palio” which likely indicates that Cosimo is a racehorse.
Next, a brief cutscene will play where the player is introduced to a pretty lady. Generally, the dialogue will be polite yet thoughtful. The young woman will depart from the conversation with Enzo curtly staying behind. Suddenly, he’ll change his mind to follow her with the player also being tasked with returning to Cesare.

From there, a miss-able dialogue icon will appear at a man named Gaspare. A mysterious figure can be seen walking away from him. To Gaspare, Enzo will ask about the young woman although Gaspare will appear to be purposely obtuse, giving a non-answer. Enzo takes the hint and continues to Cesare. Finally, Cesare informs Enzo that he has earned his meal for the day and ordered to clean up before dinner.
Attend Dinner
Next, the player is now prompted to ‘Get ready for the meal’. There will be no more points of interest other than a priest standing in front of Enzo’s living quarters. Father Ciccone welcomes Enzo to the villa. Also, the priest also offers his condolences for yesterday’s mine collapsing. Father Ciccone remarks about how generous Signor Torrisi is and how pleased that Enzo found refuge at the villa.
The historical significance of a Catholic priest’s appearance in Mafia: The Old Country is a complex one. Father Ciccone’s character underscores the time period and adds a layer of realism to the story. It should go without mentioning that the Mafia historically “borrowed” religious symbolism for their rituals and allegedly invoked the protection of Saints in their activities.
A note and a gift will be waiting for the player in Enzo’s quarters.

Charms Explained
Every Sans Celeste statue you find grants a charm. Naturally, each charm provides bonus gameplay effect when equipped. There are two types of charms, medallions and beads. Medallions are rarer and have stronger effects. Unlock more charm slots by upgrading your Rosary. Accordingly, to equip a charm, navigate the charm screen on the pause menu.
Now, Enzo nervously approaches the table as a cutscene will ensue where Enzo thanks Luca for the opportunity. Luca advises Enzo to thank Torrisi instead by working hard and staying out of trouble. Finally, we also learn the young lady from earlier is the Guest of Honor at tonight’s dinner as well as her name.
