Resident Evil Requiem: How to Find Artificial Heart and Lungs

It requires you to locate two separate artificial organs hidden across the West Wing of the Rhodes Hill Care Center: the Artificial Heart and Artificial Lungs. You must find them before you can claim the Level 2 ID Wristband needed to access the Isolation Ward.

Both organs require prerequisite items and backtracking. Furthermore, you must solve a multi-step combination puzzle to unlock them.

1. What Is the Organless Corpse Puzzle?

In the Examination Room on the ground floor of the East Wing, Grace finds a corpse strapped to a table with its chest cavity empty—missing both a heart and lungs. A computer terminal beside the body confirms that two specific organs are required: the Artificial Heart and the Artificial Lungs.

The reason why this specific corpse is crucial is that it is wearing a Level 2 ID Wristband on its wrist, and that wristband is the only way to unlock the Isolation Ward—a mandatory story-critical area that also contains the last Quartz needed to open the main hall Quartz door.

There is no way to remove the wristband without completing the puzzle.

When you return to the Examination Room with the organs, new enemies will have spawned inside that were absent on your first visit. Zombie doctors will be active in the Treatment Room, and once Blister Heads start to appear in the Care Center, a Blister Head will almost always spawn nearby.

Clear the room before interacting with the corpse, and note that the Hemolytic Injector makes no sound and is perfect for a stealthy approach.

2. Prerequisites Before You Start

You cannot collect either organ until the following items are in Grace’s possession or have done so:

  • West Wing Access Keycard: required to access all West Wing doors.
  • Level 1 ID Wristband: found in the hallway between the Lead Researcher’s Office and the Conference Room on the East Wing 2nd floor. Required for both the Garage and the Custodian’s Office.
  • Corrosive: found in the Conference Room. Required to dissolve the padlock on the Sun Puzzle Box in the Lead Researcher’s Office.
  • Sun Puzzle Box: solving the Lead Researcher’s Office Sun Puzzle Box (code: Star → Sun → Moon → Sun) unlocks the Sun Quartz and the Leatherbound Notebook inside, which contains the file “Unlocking the Organ Transport Box,” the solution to opening said box later on.

3. How to Get the Artificial Heart

Location: Cold Storage — Ground Floor, West Wing

The Cold Storage room is found at the end of the West Wing corridor and is identified by its distinctive door with a large red wheel handle. Attempting to turn the wheel by hand causes it to snap immediately, and you need a tool to force it open.

Unlock the Cold Storage

  1. With the Level 1 ID Wristband equipped, go through the locked metal gate in front of the Kitchen door on the West Wing ground floor.
  2. Enter the white door on the left into the Custodian’s Office. The Save Point typewriter is in this room—make a manual save here before proceeding.
  3. Find the Wrench on the desk beside the typewriter.
  4. Backtrack to the Cold Storage door with the broken red wheel handle. Use the Wrench on the door to force it open.
  5. Enter Cold Storage and collect the Artificial Heart—it is hanging on a hook in the middle of the room.

Note: During the West Wing section at this point, the Chef patrols nearby (unless killed previously). If you do not have Requiem ammo to deal with him, stay crouched and take alternate routes through doorways to avoid triggering his attention while heading to and from Cold Storage.

4. How to Get the Artificial Lungs

Location: Organ Transport Box — Garage, Ground Floor, West Wing

The Artificial Lungs are not left open, but are locked inside the Organ Transport Box in the back of a van in the Garage. Retrieving them requires both physically reaching the Garage and solving the box’s combination lock using instructions from the Leatherbound Notebook.

Reach the Garage

  1. From the Custodian’s Office, continue through the blue door directly ahead (not left where the Wrench was).
  2. Proceed straight, then turn right. The Garage is tucked past the Kitchen on the West Wing ground floor and easy to miss if you are not exploring thoroughly.
  3. Use the Level 1 ID Wristband to unlock the Garage door.
  4. Inside the Garage, find the Organ Transport Box in the back of the open van and pick it up.
  5. A brief cutscene plays, and immediately afterward, a zombie driving a bulldozer attempts to run Grace down. Shoot the driver and the core right below the driver’s seat to stop the vehicle before it can pin you against a wall.

Solve the Organ Transport Box Combination

The Organ Transport Box has a multi-step combination lock on its back panel. Inspecting the box from your inventory rotates it to reveal the puzzle interface. The solution comes from the Unlocking the Organ Transport Box file in the Leatherbound Notebook, obtained from the Lead Researcher’s Office Sun Puzzle Box, as noted in the prerequisites above.

To open the Organ Transport Box, follow this exact order:

  1. Turn the circular dial to Hand (left position)
  2. Adjust the long switch so it sits in the middle position
  3. Press buttons 2 and 5 downward so they are flush, while buttons 1, 3, and 4 remain raised
  4. Turn the circular dial back to Auto (right position)

Completing all four steps in order causes the Organ Transport Box to click open, revealing the Artificial Lungs inside.

5. Completing the Organless Corpse Puzzle

With both the Artificial Heart and Artificial Lungs collected, return to the Examination Room on the ground floor of the East Wing to continue your journey in Resident Evil Requiem.

  1. Clear the Examination Room of any zombie doctors that have spawned since your first visit.
  2. Interact with the open chest cavity of the corpse on the slab. Insert both organs. The order does not matter, only that both are placed.
  3. A cutscene triggers. The corpse reanimates and breaks free from its restraints.
  4. Kill the reanimated corpse. Aim for the head and watch for erratic zombie strikes—it moves more aggressively than a standard enemy.
  5. Once it falls, loot the Level 2 ID Wristband from its wrist.

The Level 2 ID Wristband unlocks the Isolation Ward, a story-critical area and the only route to the Security Office, which contains the final Quartz stone needed to open the Quartz door in the Care Center’s main hall.
